Posted by: David Patton Medication errors are problematic specifically in children for a variety of reasons, many drugs are formulated for adults and healthcare settings do not have reference materials designed specifically for children. Recommendations from the Joint Commission to prevent pediatric medication errors include standardizing protocols for pharmacy ordering and timing of medications, using oral syringes for oral medications to avoid inadvertent intravenous administration, and using error proofing technology. Another important source of errors is illegible hand writing.
